Surgical Wound Care at Home in Lahore

The incision comes home with you, and so does the responsibility for it. A nurse redresses it to your surgeon’s instructions, empties and charts any drain, and checks the whole wound line rather than the part showing above the dressing.

Surgical Wound Care at Home

We start from your discharge summary and the surgeon’s wound instructions, including when dressings should be changed and when the wound may get wet. The nurse cleans and redresses the incision, empties and measures any drain, records the output and checks temperature. If stitches or staples are due out, removal can be arranged on your surgeon’s advice.

Services Include

Goal

To support clean, uncomplicated healing of surgical wounds at home, with early warning of any problem and clear updates for your surgeon.

The First Three Weeks After Discharge

Most incisions behave. The ones that do not usually declare themselves between the fifth and the twelfth day, which is exactly when the patient is at home and the surgeon is not looking. A collection of fluid under the skin, a corner that gapes, a stitch line that turns from pink to angry red: each is manageable if it is caught, and each is why somebody should be checking the whole length.

Drains, and What They Tell You

A drain is not just a bag to empty. The volume and the colour matter: output that was falling and then rises again, or fluid that turns from straw to cloudy, is information your surgeon needs. The nurse measures each time, charts it, keeps the tubing from kinking or pulling, and checks the skin where it exits for leakage and redness.

Moving Without Opening the Wound

Abdominal and caesarean wounds give way under strain more often than under dirt. A pillow held against the wound when coughing, sneezing or sitting up takes most of that strain off. Rolling onto a side and pushing up with the arms is safer than folding straight upwards. The nurse shows the patient and whoever helps them, because this is where lifting usually goes wrong.

After Which Operations

Wounds We Follow After Discharge

We begin from your discharge summary and the surgeon’s wound instructions, not from a general dressing routine.

Caesarean Sections

Dressing and stitch checks for a mother who must still lift a baby.

Abdominal Surgery

Hernia, gallbladder and bowel incisions, with drains where they are in.

Orthopaedic Wounds

Hip, knee and fracture incisions dressed around a cast or brace.

Cancer Surgery

Longer wounds and drains after breast, bowel or other tumour surgery.

Cardiac Surgery

Chest and leg wounds after bypass, kept dry and watched.

Higher-Risk Patients

Diabetes, smoking or extra weight, where wounds break down more often.

Post-Op Routine

Everything on this page is secondary to what your surgeon wrote. We copy the instructions onto the care record, follow them, and raise anything that departs from them. A nurse who improvises on a surgical wound is a problem, not a help.

Each Dressing Change

  1. Instructions read — the discharge note sets the dressing type, the interval and when the wound may get wet.
  2. Sterile field laid — the pack is opened and the old dressing lifted without dragging on the suture line.
  3. Full length checked — every centimetre of the incision is inspected for gaping, redness or discharge, not only the sore end.
  4. Drain handled — emptied, measured, charted and resecured where one is in place.

Reported — temperature, pain score and anything new go to your surgeon before the next clinic date.

Where We Call the Surgeon

Fever, pus or a bad smell, pain that climbs instead of fading, a wound edge that separates, or a drain that suddenly stops or floods. We telephone the surgeon the same day. Heavy bleeding means hospital now.

Surgical Wound Care for Older Patients

Slower closure, more to go wrong

An eighty-year-old’s incision takes longer to knit and is more likely to break down, and the patient may not mention pain until it is severe. Our nurses check the wound line while the patient is being helped to wash, so it is seen daily rather than at the next clinic, and they watch the chest too, because a patient who will not cough is a patient who will not clear it.
Recovery after an operation is more than the wound. Help with washing, moving and medicines in the first weeks sits under post-surgery patient care, and regaining strength and walking distance comes under post-surgery physiotherapy.

FAQs

Surgical Wound Care FAQs

Questions about looking after a surgical wound at home.

Do we need visits or a nurse on shift?

Most post-operative wounds need a visit, not a shift: once a day at first, then every second or third day. The exception is the first week after major abdominal or cardiac surgery, where families often book a night nurse as well, because getting up and coughing is hardest then.
One nurse per dressing. Continuity helps, because the question is always whether the wound looks better or worse than yesterday, and a surgeon’s instruction that applies on day four may not apply on day nine. Both are written on the record for whoever comes.
The discharge summary, the dressing products your surgeon named, and the prescribed painkillers, which should be taken before the dressing change rather than after. A firm pillow for coughing is worth having. The nurse brings the sterile pack, gloves and a measuring jug for drains.
Yes, and a wound that has started to leak through the dressing should not wait until morning. Call with the operation, the date and what you can see. Heavy bleeding, a wound that has opened or a very high fever means hospital, and we will tell you so.
Yes, and it is a common request after caesarean or breast surgery. Ask at booking so it is fixed for the whole course rather than negotiated at the door. Where a male patient needs a groin or abdominal wound dressed, many families ask for a male nurse.
Per visit, with the rate depending on the wound, whether drains are involved and how long the change takes. Dressing packs your surgeon specified come from the pharmacy. The coordinator quotes the full course after reading the discharge summary, before anyone is sent.
The coordinator arranges cover for that visit. The relief nurse reads the surgeon’s instructions and the chart before she opens anything, so the dressing type and the drain routine do not change just because the face has.
Yes, and they are the ones assigned after cardiac surgery, after long stays in critical care, or where a patient is home with a tracheostomy as well as an incision. They are booked ahead, so send the discharge summary as early as you can.
