Doctor Consultations

Post-Hospital Follow-Up at Home in Lahore

A doctor visits in the first days after discharge, reads the discharge summary line by line, checks the patient against it and sorts out the medicines. Most trouble in those first weeks comes from confusion, not from the illness itself.

Discharge Summary Read Properly

Medicines Reconciled

Wounds and Lines Checked

Warning Signs You Can Keep

Post-Hospital Follow-Up

Follow-Up Care After Discharge

Ideally, call us before your loved one leaves hospital so we can plan the first visit. The doctor reviews the discharge papers line by line, checks every medicine against the prescription and examines your loved one. Further visits are scheduled around recovery, test results and hospital clinic dates, and we coordinate nursing, physiotherapy or lab tests if they are needed.

Services Include

Goal

To make the move from hospital to home safer, with medicines, warning signs and next steps clearly understood by your loved one and family.

Which Patients Need a Review After Discharge?

Anyone who came home with more medicines than they went in with. That is most patients after a heart attack, heart failure, a stroke, pneumonia, major surgery, a kidney admission or a diabetic crisis. It matters most for elderly patients on six or eight tablets, for anyone sent home with a drain, catheter or feeding tube, and for families who left the ward holding papers nobody had time to explain.

What the Doctor Goes Through

The discharge summary first, read aloud if that helps. Then every box, strip and bottle in the house laid out on the bed and matched against it, which is where the duplicates and the quietly stopped tablets turn up. Then the patient: blood pressure, pulse, temperature, oxygen, sugar, the wound, the drain, the catheter, the swelling, the walking. Then the results nobody has chased.

When to Go Back Instead

A follow-up visit is a review, not a readmission. If the doctor finds a wound that is breaking down, a fever with low blood pressure, oxygen that has dropped, a swollen tender calf, chest pain or new confusion, the answer is the hospital that discharged your relative, not a prescription at home. We will tell you that straight away.

Who Needs It

Discharges We Follow Up Most Often

Call us before discharge day if you can. The first review works best within about seventy-two hours of coming home.

After Cardiac Admissions

Heart attack and heart failure discharges, where doses change week by week.

After Major Surgery

Wound, drain and pain review once the surgical team is behind you.

After Stroke

Blood pressure, swallowing, mood, and whether therapy has actually started.

After Pneumonia or COPD

Oxygen, inhaler technique and whether the cough is settling.

Elderly Patients on Many Medicines

The group where a medicine reconciliation visit prevents the most harm, and the most readmissions.

Home With Tubes or Drains

Catheters, feeding tubes and drains checked, with nursing arranged where it is needed.

Our Approach

We would rather do one careful visit early than three worried ones later. The doctor reads the hospital’s plan before arriving, so the appointment is spent on the patient and the medicine box, not on working out what happened in the ward.

From Ward to Home

  1. Call before discharge — on 03176364604, so the first visit is booked for the day your relative comes home.
  2. Send the papers — discharge summary, operation notes and the last prescription, by WhatsApp.
  3. First review at home — within the first few days, with every medicine in the house laid out.
  4. A written plan — doses, dates, pending tests, and the warning signs that mean going back.

Then as recovery needs — further reviews, and surgical wound care or post-surgery physiotherapy where the doctor recommends them.

Where Recovery Usually Slips

Not in the operating theatre, but in the week afterwards. Two tablets doing the same job. One that was stopped in hospital and started again at home. A result nobody read. A warning sign nobody recognised.

Post-Hospital Reviews for Elderly Patients in Lahore

The first three weeks matter most

Hospitals in Lahore discharge patients earlier than they used to, which is often right for the patient and hard on the family. An older relative comes home tired, on new tablets, with a follow-up clinic date three weeks away and no clear idea what to do in between. A doctor’s visit in that gap is not a luxury. It is the part of the treatment that usually gets left out.
Doctors review discharged patients at home across DHA, Gulberg, Johar Town, Model Town, Bahria Town and the rest of Lahore. Where daily help is also needed, see post-hospitalization care and daily and per-visit home nursing.

Support Through the Recovery

The doctor’s plan often needs other hands to carry it out.

Post-Surgery Patient Care

Daily help while a surgical wound heals.

Stroke Rehabilitation

Therapy that should start soon after discharge.

Bedridden Patient Care

Turning and hygiene for patients still in bed.

Elderly Nursing Care

Nursing for older patients during recovery.

Doctor Home Visit

A one-off visit if something changes between reviews.

Post-Hospital Follow-Up

The service on this page, booked per review.

Blood Sample Collection

Pending hospital bloods collected at home.

IV Drip Administration

Antibiotics and fluids continued at home, as prescribed.

Arranging the Review

Four steps, ideally begun before your relative leaves the ward.

FAQs

Post-Hospital Follow-Up FAQs

What families ask about doctor reviews after a hospital stay.

How soon after discharge should the first review be?

There is no shift pattern for this service. It is a booked appointment, and the first one is best held within about seventy-two hours of coming home. After that the doctor sets the interval: weekly while a wound or a dose is changing, fortnightly or monthly once things are steady.
One doctor does the review. If the plan needs daily injections, dressings or tube care, a nurse comes separately on the days that work is due, and the doctor and the nurse work from the same written plan rather than two different ones.
Every medicine in the house, including the ones you stopped and the ones from before the admission. The discharge summary, operation notes and any reports. And a plain account of what has actually been happening at home: sleep, appetite, bowels, pain, and how far your relative can walk.
Yes. If your relative is already home and something is worrying you tonight, a doctor can usually come within a couple of hours. But if the worry is fever with shaking, breathlessness, a leaking wound or confusion, go back to the hospital that discharged them, or call 1122.
Yes. Tell us when you book. For a review involving a surgical wound on the abdomen, groin or breast, many families ask for a doctor of the same gender as the patient, and we will match that where someone is available in your area.
The fee is per visit and depends on your area and the time of day. A course of reviews is not sold as a package, so you are not paying ahead for visits the recovery may not need. Nursing, physiotherapy and tests are quoted separately.
Call the coordinator, not the doctor. Another doctor can take the visit, because the discharge papers and the earlier notes are on the file, so you will not be asked to tell the whole story again from the admission onwards.
For patients discharged from intensive care we will say honestly what a visiting doctor can and cannot manage at home. Close monitoring, weaning off oxygen or a tracheostomy needs nursing cover alongside the reviews, and some patients are sent home too early and need to go back. We will tell you which.
