Doctor Consultations

Chronic Disease Consultation at Home in Lahore

Diabetes, blood pressure, heart failure and COPD are managed between the crises rather than during them. A doctor reviews your relative at home or by video at a set interval, looks at the readings and the medicines together, and adjusts the plan.

Chronic Disease Consultation

Long-Term Condition Care at Home

The first consultation builds a complete picture: diagnoses, medicines, recent tests and targets for sugar, blood pressure or weight. After that, reviews are scheduled as often as the doctor recommends, mixing home visits with video check-ins. Between reviews, your family can share readings with us, and the doctor advises when tests such as average blood sugar, kidney function or cholesterol are due.

Services Include

Goal

To help keep long-term conditions steady and well managed at home, reducing the risk of complications and avoidable hospital admissions for your loved one.

Which Long-Term Conditions These Reviews Cover

Diabetes and high blood pressure most often, then heart failure and rhythm problems, COPD and asthma, chronic kidney disease, thyroid disorders, arthritis and Parkinson’s disease. Many of our patients have three of these at once and take eight or nine tablets a day, prescribed over the years by different doctors who never saw each other’s notes. Pulling that into one plan is usually the first useful thing a review does.

What a Review Looks At

The logbook first: sugar readings, blood pressure numbers, weight, and what was happening on the days they went wrong. Then symptoms, sleep, appetite, mood and side effects, because a tablet that causes dizziness gets quietly abandoned. Then an examination aimed at the condition, feet and pulses in diabetes, ankles and chest in heart failure. Then the medicines, and the date of the next review.

What These Reviews Cannot Promise

Regular review helps keep a long-term condition steadier and catches change earlier. It does not cure diabetes, reverse kidney damage or guarantee that nobody goes into hospital again. Some conditions progress whatever is done well. What a careful plan can do is make the drift slower, the surprises fewer, and the decisions clearer for your family.

Who Needs It

Conditions We Review Month by Month

The first consultation is longer than the rest, because it builds the picture nobody has yet written down in one place.

Type 2 Diabetes

Sugar patterns, feet, kidneys, and whether the tablets still fit.

High Blood Pressure

Home readings reviewed against the doses, not clinic one-offs.

Heart Failure

Weight, ankles, breathlessness and diuretic doses, reviewed often.

COPD and Asthma

Inhaler technique, flare-up plans, and when oxygen is appropriate.

Chronic Kidney Disease

Which everyday medicines are no longer safe at this level of kidney function.

Several Conditions Together

The commonest case: three diagnoses, eight tablets, and no single overall plan.

Our Approach

Continuity is the whole point of this service. We keep the same doctor on the file wherever we can, because the value is not in any single review but in a doctor who remembers what the readings looked like three months ago.

How a Plan Runs

  1. First consultation at home — diagnoses, every medicine, recent tests, and what matters to your relative.
  2. Targets and a logbook — what to measure, how often, and the numbers we are working towards.
  3. Reviews booked ahead — monthly or fortnightly at first, mixing home visits with video calls.
  4. Tests when due — average sugar, kidney function and cholesterol, with samples collected at home.

Between reviews — send the readings on WhatsApp, and the doctor says whether to wait or to be seen.

Why Intervals Beat Emergencies

Most admissions for diabetes or heart failure are the end of a slow drift that was visible in the readings weeks earlier. Somebody has to be looking at those readings regularly for that to be any use.

Diabetes and Blood Pressure Reviews at Home

For parents who stopped attending clinic

There is a pattern we see often in Lahore. A parent was going to the hospital clinic, then the queues grew too long, then a visit was missed, then the test strips ran out, and now nobody knows what the sugar has been doing for a year. Nothing dramatic happened. The condition simply stopped being watched. A review at home restarts it, without the queue being the obstacle again.
Reviews take place at home across Johar Town, DHA, Gulberg, Model Town, Bahria Town and the rest of Lahore, or by video in between. For readings taken between reviews, see blood sugar monitoring and blood pressure monitoring.

Services That Support the Plan

Monitoring and tests that keep a long-term plan honest.

Post-Surgery Patient Care

Recovery support if an operation interrupts things.

Stroke Rehabilitation

Therapy after a stroke, alongside the reviews.

Bedridden Patient Care

Daily care for patients with advanced illness.

Elderly Nursing Care

Nursing for older patients between doctor reviews.

Doctor Home Visit

An extra visit when a condition flares.

Post-Hospital Follow-Up

Picking the plan back up after an admission.

Blood Sample Collection

Average sugar, kidney function and cholesterol, collected here.

IV Drip Administration

Infusions given at home when a doctor prescribes them.

FAQs

Chronic Disease Consultation FAQs

Questions families ask about long-term condition reviews at home.

How often are the reviews?

Reviews are appointments rather than shifts. Monthly is the usual starting interval, fortnightly while doses are being changed, and every two or three months once things hold steady. The doctor sets it and explains why, and the date of the next review is agreed before they leave the house.
One doctor, kept the same wherever we can. If insulin needs giving, feet need dressing or readings need taking between reviews, a nurse comes for that work and follows the doctor’s written plan rather than a separate one of her own.
A logbook, even a school exercise book, with the sugar or blood pressure readings and their dates. Every medicine box in the house. The last set of blood results. And a working glucometer or blood pressure machine with strips that have not expired, which catches out more families than you would expect.
A first consultation can usually be arranged within a day or two, and for a stable long-term condition that is soon enough. If a reading is frightening you tonight, say so when you call. Very low sugar with confusion, chest pain or severe breathlessness means 1122, not an appointment.
Yes, tell us when you book. It matters more in this service than families expect, because diabetic foot checks and heart failure examinations are repeated at every review, and a patient who is uncomfortable will start finding reasons to skip them.
Reviews are charged per consultation, and video check-ins cost less than home visits, which is part of why we mix them. There is no annual plan to buy up front. Tests, monitoring visits and medicines are quoted separately so you can see what each one costs.
Call 03176364604 and we will rebook, usually within a few days. For a planned review a short delay is safer than sending a doctor who has not read the file, because the whole value of this service is continuity.
Not through this service, because it is a doctor’s review rather than nursing care. Chronic conditions do flare, though. If your relative is admitted, our doctors can pick the plan up again afterwards through post-hospital follow-up, with the hospital’s changes folded into it.
