Patient Care

Long-Term Patient Care at Home in Lahore

Care arranged by the month rather than the week. One primary carer who comes to know the patient, a named relief carer for days off, a written plan a senior nurse reviews at set intervals, and records that make slow change visible early.

Long-Term Patient Care

Long-Term Patient Care at Home

We assign a primary caregiver, plus a regular relief carer for days off, so the patient sees familiar faces. A senior nurse reviews the plan at set intervals and whenever the condition changes, coordinates nursing visits, and liaises with your doctor or physiotherapist. Families receive regular updates and can raise concerns with our care coordinator.

Services Include

Goal

To give a patient with a long-term condition consistent care at home that keeps them as comfortable, safe and independent as their health allows.

When Care Becomes Part of Daily Life

Long-term care is for conditions that are not going to resolve in a few weeks. That includes life after a major stroke, paralysis and spinal injury, Parkinson’s disease, multiple sclerosis and motor neurone disease, dementia, advanced heart, lung or kidney disease, patients on dialysis, and people living with cancer or receiving palliative care. Families usually book it once they see the arrangement needs to last months rather than days.

Why the Same Carer Matters

A carer who has been with a patient for weeks knows how much they usually eat, which side they prefer to lie on, when the stiffness is worst and what their normal mood looks like. That is what lets them notice the day something is different. A stranger every week sees only a snapshot, and slow decline hides easily in snapshots.

Keeping the Care Plan Current

A senior nurse reviews the written plan at set intervals, and whenever the condition changes in either direction. Weight, skin, appetite, continence, mobility and mood are recorded at every shift, and the trend is read rather than the single reading. Changes go to your doctor, physiotherapist and family, so everyone works from the same picture.

Who Needs It

Conditions We Support Month After Month

Nothing here is a fixed package. The plan is written for one patient and rewritten as their condition moves.

Life After a Major Stroke

Slow recovery, with help for feeding, transfers and daily exercises.

Paralysis & Spinal Injury

Bowel and bladder routines, skin protection and positioning, every day without exception.

Parkinson's Disease

Medicines on exact timings, fall prevention and patience with slow movement.

Motor Neurone Disease

Care adjusted as swallowing, speech and breathing change.

Dialysis & Organ Failure

Fluid and diet limits, transport to sessions, and close watch on blood pressure and swelling.

Palliative Care Support

Comfort, mouth care, pain relief as prescribed and dignity, with the family kept close.

Our Approach

Long-term care fails from fatigue, not from a crisis. Carers who work too many days in a row become careless, and families who never get a break burn out. So we build a rota with relief in it from the start, and we expect you to use it.

How a Long Placement Works

  1. Full assessment — a senior nurse looks at the diagnosis, what your doctor expects, and how the household runs.
  2. Primary carer assigned — matched on skill, language and temperament, because they will be with you a long time.
  3. Relief carer named — the same second person covers days off, so the patient meets one substitute rather than five.
  4. Plan written and signed off — personal care, meals, medicines, exercises, equipment and warning signs.

Scheduled reviews — the nurse revisits the plan at agreed intervals and whenever the condition changes, and nursing visits are added or dropped to match.

Making the Arrangement Last

Families often start by trying to do everything themselves, and ask for help only once someone is ill from exhaustion. A placement planned from the beginning, with relief and rest built into it, lasts far longer.

Long-Term Care for Chronic Illness and Paralysis

A routine the patient can rely on

Looking for a long-term carer for a chronically ill relative in Lahore? Months of illness wear a household down in small ways: meals at odd hours, nobody sleeping properly, every plan provisional. A steady carer puts a shape back on the day, so the patient knows when they will be washed, fed, moved and left in peace, and the family can make arrangements that hold.
Our carers work across DHA, Gulberg, Johar Town, Model Town, Bahria Town and beyond. Many long-term patients also need home physiotherapy to keep joints moving, and a regular chronic disease consultation to review treatment.

FAQs

Long-Term Patient Care FAQs

Questions families ask when care will run for months.

Can the shift pattern change as the condition changes?

Long-term patients are rarely on the same pattern all year. Many start with a twelve-hour day shift, add a night shift when sleep becomes a problem, and move to round-the-clock cover or a live-in carer later. The pattern is agreed in advance each month and changed with notice.
The daily work is one carer at a time, with a named relief carer for days off and leave. Nursing is separate: a qualified nurse visits on scheduled days for tube feeds, catheters, wounds or injections rather than staying all day. Round-the-clock cover needs two carers, one per shift.
Over months the supplies matter more than the equipment: diapers, wipes, barrier cream, feeds and prescribed medicines kept in stock, so nothing runs out on a Sunday. A hospital bed, air mattress, wheelchair or commode makes the work safer, and the nurse will advise on what is worth buying.
A night shift can often be covered quickly, but long-term care is better started properly. We would rather do the assessment, write the plan and name your primary carer first, which usually takes a day or two. Short-term cover can bridge the gap if you need someone immediately.
Yes, and over a long placement it matters more than usual. Tell us your preference at the assessment. Where the patient needs intimate care, most families ask for a carer of the same sex, and we hold to that for the relief carer too.
Long-term care is usually quoted by the month, based on the shift pattern and whether the work needs a caregiver or a qualified nurse. The rate is agreed before the placement and reviewed when the plan changes, so a step up to night cover is never a surprise.
This is what the named relief carer is for. They already know the patient, the routine and the plan, so an absence does not become an interruption. If both are unavailable, the coordinator arranges and briefs a substitute.
Yes, though long-term patients more often need scheduled nursing than critical care. Where someone is on a tracheostomy, home ventilation or suction, an ICU-trained nurse is placed for those hours and the caregiver covers the rest of the day.
